Purpose
This service provides free non-emergency medical transportation (NEMT) to and from medical appointments for eligible clients through trusted rideshare services such as Uber and Lyft. All rides are provided on a curb-to-curb (C2C) basis. Our goal is to help remove barriers to healthcare access.
Terms of Service
- Free Service – Transportation is provided at no cost to the client.
- Scheduling – Rides must be requested at least 48 hours in advance. Same-day or last-minute requests are not guaranteed.
- Curb-to-Curb Service – All transportation is provided on a curb-to-curb (C2C) basis. Drivers will pick up and drop off at the curb of the specified addresses.
- Service Limitations – We do not provide specialized non-emergency medical transportation services, including wheelchair-accessible vehicles, bariatric vehicles, or stretcher/gurney transport.
- Availability – Service depends on rideshare driver availability, weather, and safety conditions. We reserve the right to reschedule or cancel if needed.
- Transportation Method – Rides are arranged through third-party rideshare services such as Uber or Lyft. Clients agree that by accepting the ride, they understand it will be fulfilled by an independent rideshare driver.
- Medical Care Disclaimer – Rideshare drivers are not medical professionals. They will not provide medical advice, treatment, or assistance beyond safe curb-to-curb transportation.
- Liability – While every effort is made to ensure safe and reliable service, FocusByMattnMe is not responsible for accidents, delays, or personal items lost or left in the vehicle. Clients acknowledge that Uber, Lyft, or other rideshare companies operate as independent third parties and assume responsibility under their own policies.
- Behavior Expectations – Clients must treat drivers and other passengers with courtesy and respect. Unsafe, disruptive, or inappropriate behavior may result in loss of service eligibility.
- Emergency Situations – This service is not for medical emergencies. In case of an emergency, call 911.

By signing below, I confirm that I have read and understand the terms of the free non-emergency medical transportation service. I agree to use the service responsibly and acknowledge that rides are arranged through Uber or Lyft on a curb-to-curb basis. I understand that specialized transportation (wheelchair-accessible, bariatric, or stretcher/gurney) is not available through this service. I also understand that this is a support service, not a professional medical or emergency service.
