Disclosure Agreement for Free Medical Non-emergency Transportation Service

Purpose

This service provides free transportation to and from medical appointments for eligible clients. Our goal is to help remove barriers to healthcare access.

Terms of Service

  1. Free Service – Transportation is provided at no cost to the client.
  2. Scheduling – Rides must be requested in advance. Same-day or last-minute requests are not guaranteed.
  3. Availability – Service depends on driver availability, weather, and safety conditions. We reserve the right to reschedule or cancel if needed.
  4. Transportation Method – Rides may be provided directly by volunteer/community drivers or arranged through a third-party service such as Uber or Lyft. Clients agree that by accepting the ride, they understand it may be fulfilled by an independent rideshare driver.
  5. Medical Care Disclaimer – Drivers (volunteer or rideshare) are not medical professionals. They will not provide medical advice, treatment, or assistance beyond safe transportation.
  6. Liability – While every effort is made to ensure safe and reliable service, FocusByMattnMe is not responsible for accidents, delays, or personal items lost or left in the vehicle. Clients also acknowledge that Uber, Lyft, or other rideshare companies operate as independent third parties and assume responsibility under their own policies.
  7. Behavior Expectations – Clients must treat drivers and other passengers with courtesy and respect. Unsafe, disruptive, or inappropriate behavior may result in loss of service eligibility.
  8. Accessibility – Clients must inform us in advance if they use mobility aids (e.g., wheelchair, walker) so proper arrangements can be made.
  9. Emergency Situations – This service is not for medical emergencies. In case of an emergency, call 911.

By signing below, I confirm that I have read and understand the terms of the free transportation service. I agree to use the service responsibly and acknowledge that it may be provided by volunteers, community drivers, or arranged through Uber/Lyft. I also understand that this is a support service, not a professional medical or emergency service.
